When you share your personal information with any company, you have a right to expect that information to be treated with total confidentiality.

At Basis Biologic, we know that you care how your personal information is used and we appreciate that you trust us to do that carefully and sensibly.

This Privacy Policy is designed to help you understand how we collect and use your personal information. We want you to make informed decisions when using our website and any features on them, including any registration and communication services.

This Privacy Policy applies to all the pages on our sites linking to this Privacy Policy and any features of them.  However, it won’t apply to any third-party sites which are linked to our sites. We recommend that you read the privacy notice of any such sites that you visit as we are not responsible for them.

Some information on how data is sometimes collected from you includes:

  • On blog posts: your name, email and URL are collected on comments. The purpose of collecting this information is to display to other viewers to identify you as the person who left the comment. This allows for open dialogue and conversation between commenters. Your name and URL (if submitted) will be displayed, but your email will not. The email is saved in case we need to contact you directly about the comment you left. You can choose to comment on the blog as a guest and only your name will be required. If you'd like your comment to be deleted, please contact us to request this action.
  • When you fill out a "contact" form on the Contact page, your email and name are collected when you submit the form to get in touch with me. This data is collected so that I can email you to answer any questions you have, and to initiate a conversation about any of the services that you're interested in.
  • Acuity Scheduling Software is used for scheduling through the Basis Biologic website. A deposit is required for all PRP procedures, and payment is taken through the Acuity website. By setting up an appointment and/or paying through this system, you agree to abide by their Terms and Conditions.  For further information about the type of personal information that Acuity collects, please refer to their Privacy Policy. 
  • If you sign up for the Basis Biologic newsletter, it is through MailChimp, a reputable company* which provides online platforms that can be used to create, send, and manage emails. In providing this service, MailChimp may collect personal information, such as distribution lists which contain email addresses that Members have sent, or intend to send, emails to, and all information relating to those email addresses. For further information about the type of personal information MailChimp collects, please refer to their Privacy Policy.

MailChimp will use this information for the purpose of hosting the online platform to enable members (such as Basis Biologic) to create, send and manage emails. MailChimp will also use this information to measure their Email Campaign’s performance and to improve the features for specific segments of customers, evaluate your use of our website, compiling reports on website activity for website operators and providing other services relating to website activity and internet usage. MailChimp may transfer this information to third parties where required to do so by law, or where such third parties process the information on MailChimp's behalf. MailChimp uses cookies, Web Beacons and Flash player code to collect information about when you visit the website, when you use the services, your browser type and version, your operating system, and other similar information.

We are required to inform you that by subscribing to our e-newsletter:

  • you consent to your personal information being collected, used, disclosed and stored as set out in MailChimp’s Privacy Policy and agree to abide by MailChimp’s Terms of Use;
  • you understand and acknowledge that this service utilizes a MailChimp platform; and

You can opt out of our e-newsletter if you chose the ‘unsubscribe’ service provided by MailChimp in every email, or contact Basis Biologic. You can also disable or refuse cookies or disable Flash player; however, you may not be able to use the services provided by MailChimp if cookies are disabled. Third-party disclosure

We do not sell, trade, or otherwise transfer to outside parties your Personally Identifiable Information.

 

Third-party links

Our website may contain links to other websites which are outside our control and are not covered by this Privacy Policy (“Third-Party Websites”). If you access Third-Party Websites using the links provided, the operators of such sites may collect information from you which will be used by them in accordance with their own privacy policies, which may differ from ours. We encourage you to review the privacy policies of those Third-Party Websites so that you understand if/how they collect and/or use information from you or your computer. In the bottom left hand corner of the site, there is a button that says continue that includes the information that the site collects cookies. If you do not click on continue, sensitive cookies will not be collected. If you choose to click on continue, this site may record information in relation to your use of the site, such as pages visited, searches, referring URL and timestamp. This information is anonymous and only used for essential site functioning. If you would like to revoke the usage of cookies, you can either open this website in incognito mode which doesn't allow cookie usage, or you can close out of it, delete your browser's cookies and reopen the website without clicking on the "continue" button in the left hand corner for the cookies disclosure.
